The horizontal tunnel is cut very straight into the rock, however there is a slight dogleg, this and the evidence of the cut marks on the walls (see next photo) show that the tunnel was cut by teams working from each end, who met in the middle. This is a stupendous feat of engineering, one recorded in similar circumstances in Jerusalem by those who cut the tunnel (on the Siloam Inscription).
